Key Responsibilities

• Evaluate and treat patients in accordance with physical therapy standards of practice
• Develop and implement individualized treatment plans to support patient recovery and functional goals
• Document patient progress and maintain accurate clinical records
• Collaborate with interdisciplinary care team to coordinate patient care
• Maintain valid state Physical Therapist license and adhere to all facility compliance requirements
• Present valid state or federal issued photo identification and wear a photo badge at all times while on site

Required Qualifications

• Active New Jersey Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program
• Ability to work Monday through Friday day shifts
• Must provide valid state or federal issued photo identification upon arrival at the facility

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Experience working in a registry or contract staffing capacity
