Role Description The Admissions RN is a full-time, on-site role based in Fall River, MA, responsible for coordinating and completing hospice admissions for eligible patients. This team member conducts comprehensive nursing assessments, explains hospice services and philosophy to patients and families, and obtains necessary consents and documentation. The Admissions RN collaborates closely with physicians, clinical staff, and interdisciplinary team members to develop initial plans of care and ensure a smooth transition into hospice services. Daily activities include symptom and pain assessment, verification of eligibility criteria, accurate documentation in the electronic medical record, and communication with referral sources and caregivers. The role also involves patient and family education, advocacy, and ongoing coordination to support a dignified and comfortable start to hospice care. Qualifications Current, unrestricted Registered Nurse (RN) license in the state of Massachusetts; hospice or home health experience strongly preferred. Proficiency in comprehensive patient assessment, care planning, and symptom management in hospice or palliative care settings. Strong communication and interpersonal skills for educating patients, families, and caregivers about hospice services and coordinating with interdisciplinary teams. Ability to accurately document clinical encounters, maintain detailed records in electronic health systems, and follow regulatory and organizational protocols. Demonstrated commitment to compassionate, patient-centered care, including cultural sensitivity and respect for diverse backgrounds and beliefs. Reliable organizational and time-management skills to manage multiple admissions, prioritize tasks, and respond to changing patient needs. Valid driver’s license and reliable transportation, if travel to patient homes or facilities is required. Basic life support (BLS) certification; additional hospice or palliative care certifications are a plus. Experience collaborating with physicians, social workers, chaplains, and other healthcare professionals in an interdisciplinary care model.
