The Care Case Coordinator creates the unique care plan, manages the care, and provides constant feedback to clients, clients representatives and caregivers. They work closely with our office team and have excellent communication and problem-solving skills. Upbeat Personality Current, unrestricted California LVN license Graduation from accredited practical or vocational nursing program Industry Experience, preferred Strong Computer Skills Strong Organizational & Communication Skills Pass Live Scan Fingerprint Clearance (LIC 9163) Valid driver’s license and proof of insurance required Cell phone with Internet Access Proof of Negative TB results Willing to Work One Weekend a Month as Back-Up On Call Fun Work Environment Supportive Team and Management Anthem Blue Cross or Kaiser Health Insurance United Concordia Dental Insurance Vision Insurance Matching 4% 401K Plan Career Advancement Opportunities Salary $90,000+ based on experience.
