As a full-time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. This role includes accountability for Plan Medical Management, comprehensive health risk assessments, chronic disease management, and care coordination. The NP practices within the scope of state regulations, clinical protocols, and collaborative practice agreements, with a strong focus on post-acute care, provider-based care, and value-based clinical performance./ph3Qualifications/h3p 2 years of experience as a Nurse Practitioner in adult, geriatric, or family practice settings preferred./pp Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease/pp Skilled in identifying risk factors, unmet needs, and gaps in care for senior populations/pp Proficient in medication reconciliation and managing clinically complex patients/pp Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred)/pp Active RN and NP licensure in the state of practice; board certified (AANP or ANCC)/pp Current CPR certification/pp Highly organized, detail-oriented, and committed to ongoing learning and professional development/ppPreferred/pp Geriatric certification (or willingness to obtain)/pp Bilingual (English/Spanish)/pp Familiarity with community resources and behavioral health needs/pp Experience serving vulnerable, low-income, or disadvantaged populations/ph3Essential Duties/h3ulliPerform comprehensive health risk assessments for assigned Members/liliCollaborate with physicians, case managers, social workers, caregivers, and facility staff to coordinate care/liliIdentify quality improvement opportunities and develop individualized care plans/liliReview and interpret medical, surgical, and social histories/liliPerform appropriate screening procedures and interpret diagnostic data/liliRecommend and implement treatment plans, including prescribing medications, ordering tests, and providing wellness counseling/liliReconcile medications and ensure safe, appropriate use/liliRefer Members to PCPs, case management, or disease management programs as needed/liliMaintain compliance with HIPAA, confidentiality standards, and PHI regulations/liliIdentify unmet needs and initiate appropriate referrals/liliDiagnose conditions and create comprehensive, evidence-based plans of care/liliAchieve clinical performance targets and quality benchmarks/liliBuild positive rapport and engagement with Members and families/liliCommunicate and collaborate with facility leadership to support Member care goals/liliParticipate in committees related to clinical guidelines, quality improvement, and compliance/liliMaintain team schedules to ensure continuity of care/liliTravel independently to assigned facilities as required/liliServe as a clinical leader within the facility, supporting provider-based care and post-acute care workflows/li/ulpTravel Requirements/pulliTravel required between assigned facilities/liliMust meet company driving standards/liliMust maintain valid drivers license and automobile insurance/liliMust be able to independently travel to all assigned locations/li/ulpPhysical Psychological Demands/pulliProlonged sitting; occasional bending, stooping, kneeling, or stretching/liliManual dexterity to operate medical equipment and office technology/liliRequires close and peripheral vision, hearing, and ability to adjust focus/liliAbility to lift and/or move up to 50 lbs/liliAbility to follow universal precautions for blood-borne pathogens/liliModerate to intense concentration required due to clinical complexity/li/ulpWork Environment/pulliCombination of administrative and clinical duties/liliOccasional exposure to variable temperatures, airborne particles, and body fluids/liliModerate noise levels/liliReasonable accommodations available upon request/li/ulpWe are an Equal Opportunity Employer.
