Field Medical Case Manager - Return-to-Work Navigator

ForzaCare in New York is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work processes. You will serve as the central coordinator among workers, medical providers, employers, and insurers, ensuring timely and transparent care management. This role is ideal for licensed RNs or CRCs, with preferred workers' comp experience and credentials such as CCM or CIRS.
