Requirements
- Current Physical Therapist license in the state of practice
- Experience in skilled nursing facility or geriatric rehabilitation preferred

Roles & Responsibilities
- Evaluate residents' mobility, balance, strength, gait, and functional limitations.
- Develop and implement personalized treatment plans to improve physical function, reduce pain, and prevent disability.
- Provide direct therapy interventions, including therapeutic exercises, gait training, and balance retraining.
- Monitor progress and adjust care plans as necessary to reach therapy goals.
- Educate residents, families, and caregivers on home exercise programs and safety strategies.
- Supervise Physical Therapist Assistants and therapy aides in accordance with licensure requirements.

Compliance / Notes
- Ensure compliance with all state, federal, and facility documentation and practice standards.
