CHC isa non‑profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction. CHC has been a critical part of Ohio’s efforts to treat and prevent substance use disorders since 1974.

We are looking for a Patient Care Liaison. The Patient Care Liaison is a cross‑functional healthcare support role responsible for enhancing patient access, supporting clinical workflows, assisting with continuity of care activities, and maintaining accurate documentation processes within a federally aligned primary care clinic environment. This position serves as a key link between patients, providers, clinical staff, and external healthcare entities.

Duties Include

• Front desk and patient access operations
• Medical assistant workflows
• Referral follow‑up and tracking
• Transportation of clients, as needed
• Continuity of care support
• HRSA Health Center Program Requirements
• Support accurate collection and documentation of required patient data elements

Minimum requirements

• High school diploma or equivalent required
• Exceptional attention to detail
• Strong communication and customer service skills
• Ability to work independently and collaboratively
• Understanding of patient confidentiality standards
• Must have a valid driver's license and must pass a background check and drug test
