Social Worker - Stroke/Neurology (PART-TIME)

Job Title: Social Worker - Stroke/Neurology (PART-TIME) The Mount Sinai Hospital is a large, comprehensive medical center. The Social Work Services department offers programs across many areas and works to meet patient needs. The social worker (0.6 FTE) will provide psychosocial services, including discharge planning, for hospitalized stroke patients on 8W/8C and other neurological patients. Responsibilities Comprehensive assessment of stroke patients, including depression screening using a standardized screening tool. The social worker collaborates with the interdisciplinary team in support of the Comprehensive Stroke Center program and is actively involved in the Respiratory Recovery Pathway. Opportunity to do program development, e.g., Performance Improvement projects and group work. Coverage includes 8W and other medicine/surgery units. Qualifications NYS LMSW or LCSW. Inpatient hospital experience preferred. Excellent organizational, collaborative, communication and short-term psychosocial assessment and intervention skills. Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast‑paced environment and coordinate resources within hospital and community networks for effective management of patient care. Experience with or interest in grief/trauma/terminal illness. Experience with or interest in crisis intervention work. Experience with or interest in program development/research opportunities. Internal applicants: Minimum one year in current position unless currently a Float in Medicine/Surgery. Compensation The salary range for the role is $85280-$101670.78 annually.
