Job Summary Under the direction of the Health Officer/Director, the Public Health Nurse provides public health nursing services to individuals, families, and population groups. Responsibilities include public health nursing actions directed toward prevention, assessment, risk reduction, and health status improvement. Activities include systemic analysis of health data, care plan development, health education and advocacy, coalition building, and collaboration with community partners. *It is the policy of Shawano County to recruit and select the most qualified persons for positions in the County's service. Essential Duties & Responsibilities Public Health Sciences Skills Perform comprehensive individual and family assessments, health history, physical assessment, growth monitoring, developmental assessment, psycho-social assessment, assessment of family functioning, assessment for substance abuse or domestic violence issues, and assessment of basic needs including food, housing, income, resources and supports, and access to health care. Provide care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups. Evaluate outcomes, effectiveness of plan, and make changes as necessary. Use public health surveillance/disease investigation methods in community outreach, screening and case finding of communicable and infectious diseases that threaten the health of the community. Investigate complaints regarding environmental health problems and hazards, and initiate enforcement or corrective actions as required. Provide education and counseling to individuals, families and community groups adapted to their unique needs, lifestyle, cultural and socio-economic situation. Obtain and complete appropriate patient consent forms, ensure confidentiality and maintain orderly medical records in accordance with public health policies and laws. Document patient and environmental assessment and intervention data in medical records using established systems, forms, databases and documentation practices. Respond to individual client emergency situations in the clinical or home setting with basic life support and first aid skills; summon other health care providers and emergency response personnel in a timely manner. Collaborate in development and delivery of programs for individuals, families and population groups that promote health and prevent disease in settings including health department sites, homes, community organizations, businesses, schools and the community. Evaluate outcomes of public health environmental and other nursing interventions; work with others to make changes as necessary. Participate in response teams to prevent, minimize, and contain adverse health events and conditions resulting from communicable diseases, chronic diseases, water quality, environmental hazards, injuries and health disparities. Community Partnership Skills Deliver targeted, culturally-appropriate information to help individuals and groups understand health promotion and disease prevention policies, regulations, and local code. Actively participate in and act as a liaison with other community agencies and professionals in collaborative efforts to address community health priorities. Advocate on behalf of vulnerable individuals and populations; participate in assessing and evaluating health care services to ensure people are informed of available programs and are assisted in utilization. Participate in the Shawano-Menominee Counties Health Department research and demonstration projects that seek to improve the health of communities and determine new ways to address health issues. Contribute to a work environment that fosters ongoing educational experiences regarding public health nursing for colleagues and community members. Data Analytics and Assessment Skills Participate in community assessment, distinguish between quantitative and qualitative data, understand incidence and prevalence data, and access basic community epidemiology data. Analyze data to identify trends, health problems, environmental health hazards, and social and economic conditions that adversely affect public health. Leadership and Systems Thinking Skills Demonstrate knowledge of applicable state professional practice guidelines, the American Nurses’ Association Scope and Standards of Practice for Public Health Nursing, and applicable federal and state laws. Adhere to OSHA standards concerning exposure to blood-borne pathogens, toxic substances, airborne pathogens, or other hazards during routine assignments or emergencies. Assume responsibility for professional growth by pursuing education, participating in professional committees, and contributing to continual improvement in practice. Coordinate activities among and between governmental agencies such as the Wisconsin Department of Health Services. Collaborate in the development of evidence-based public health nursing practices and programs with universities and other partners. Collaborate in the development, implementation and evaluation of policies, plans and programs for public health, environmental public health and nursing in Shawano and Menominee Counties. Collaborate in and contribute to performance management, quality improvement and evaluation activities. Communication and Cultural Competency Skills Utilize appropriate methods for interacting effectively with persons of all ages and diverse backgrounds, cultures, socio-economic, educational, racial and ethnic backgrounds, sexual orientations, lifestyles and physical abilities. Educate local and state policy makers and community stakeholders on public health issues. Adhere to ethical principles and department policy in the collection, maintenance, use and dissemination of data. Financial Planning and Management Collaborate on MCH, Immunization, Lead and other consolidated contract budgets. Participate in activities such as time studies that contribute to budget development and efficient resource use. Accurately track time spent on public health projects. Other Perform public health emergency response duties in response to threats consistent with job classification, training and Incident Command Structure. Actively participate in Public Health Accreditation Board (PHAB) activities. Perform other duties as assigned consistent with job classification and licensure. Work Requirements Bachelor’s degree in Nursing meeting HSS 139. Registered Nurse licensure in Wisconsin, CPR/AED certification, valid Wisconsin driver’s license or evidence of mobility.
