Integrated Behavioral Health Clinician
The Providence Community Health Centers, Inc.
The Providence Community Health Centers, Inc. seeks an Integrated Behavioral Health Clinician to provide assessments, brief therapy, and care coordination within primary care teams. The clinician conducts screenings, develops person-centered treatment plans, delivers short-term, evidence-based interventions, and manages crises for diverse, underserved patients. Working closely with medical, nursing, and care management staff, this role supports warm handoffs, closes care gaps, and connects patients to community resources while maintaining thorough EHR documentation and contributing to quality improvement and team-based care.
