About the Position

Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 hours Experience: Minimum 1 year of Certified Home Care experience; Minimum 1 year of Case Management experience License: RN License Certifications: None specified Must-Have: - 1 year Certified Home Care experience - 1 year Case Management experience - Ability to travel to assigned cases - Weekend rotations once every 6 weeks Description: All applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is required. Experience in hospice, private care, or working as a Case Manager alone does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. The position also requires performing admission processes and case management responsibilities in accordance with policies and standards of practice. The use of thorough and timely electronic documentation and interdisciplinary communications is essential. Start date is based on credentialing clearance. If not cleared by the posted start date, a rescheduled start date will be necessary.
