As a Hospital Liaison for Focus Health Network, LLC in Pittsburgh, PA, you will play a critical role in ensuring seamless communication and collaboration between our healthcare teams and hospital partners. With a focus on delivering exceptional patient care, you will serve as the primary point of contact for hospital staff, providing timely and effective solutions to meet their needs. Establish and maintain strong relationships with hospital staff, including physicians, nurses, and administrators

Provide timely and effective communication to hospital partners, ensuring efficient and accurate exchange of information

Coordinate and facilitate hospital-to-hospital and hospital-to-home transitions, ensuring continuity of care for patients

Identify and resolve issues promptly, utilizing problem-solving skills and critical thinking to find creative solutions

Collaborate with healthcare teams to develop and implement processes and procedures that promote efficient and effective care transitions

Participate in quality improvement initiatives, analyzing data and implementing changes to improve patient outcomes and satisfaction

Maintain accurate and up-to-date records, ensuring compliance with regulatory requirements and organizational standards

2 years of experience in healthcare administration, patient navigation, or a related field

Strong communication, problem-solving, and organizational skills

Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines

Strong attention to detail, with a focus on accuracy and quality

Ability to maintain confidentiality and handle sensitive information with discretion

Strong knowledge of the healthcare industry, including regulations and best practices
