Position Details Position: Ambulatory Nurse II Entity: Clinical Care Associates Department: Valley Forge Floor 3 Location: Penn Medicine Valley Forge—1001 Chesterbrook Blvd Hours: Full‑Time Job Summary The Ambulatory Nurse II colleague is an RN who practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. The colleague possesses the knowledge, skills, and attitude to effectively meet standards for competent professional nursing practice as specified in the Ambulatory Nurse II position description addendum. The colleague exhibits UPHS organizational behavioral competencies that reflect our mission and are embedded in the professional practice competency domains. The colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly. The colleague demonstrates competencies in Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork. The colleague works closely with the interprofessional health care team to facilitate coordination of care across the continuum and focuses on expanding knowledge and skills to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients. Accountabilities Evidence Based Practice and Research: Evaluate and integrate best current evidence with clinical expertise and patient and family preferences and values to deliver optimal health care and system effectiveness. Safety: Minimize risk of harm to patients, families, providers and self through system effectiveness and individual performance. Professionalism: Demonstrate a commitment to the nursing profession through lifelong learning, adherence to ANA’s Code of Ethics for Nurses, participation in a professional organization and advancing community outreach. Person and Family Centered Care: Recognize the patient (or the patient’s designee) as the source of control and experience a full partnership in providing compassionate and coordinated care based on respect for the patient’s preferences, values, and needs. Leadership: Collaborate and apply innovative, systems thinking to engage in systematic, evidence-based problem solving and decision making to promote effective changes within a complex care delivery system supporting the vision of Penn Medicine. Informatics/Technology: Utilize appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision making across the continuum. Teamwork: Engage in cooperation, coordination, and collaboration to provide safe, quality outcomes for patients within inter- and intra-professional teams, including virtual teams. Continuous Quality Improvement (CQI): Use data and QI methods to identify potential and actual problems and opportunities to provide care that is safe, timely, efficient, effective, and equitable. Performs duties in accordance with Penn Medicine and entity values, policies, and procedures. Other duties as assigned to support the unit, department, entity, and health system organization. Qualifications Diploma or ASN from an accredited school of nursing and more than 15 months of relevant professional nursing experience (required). BSN from an accredited school of nursing and more than 15 months of relevant professional nursing experience (preferred). Licenses, Registrations, and Certifications RN with state licensure (required). BLS/ACLS/PALS per specialty (required). Certification in specialty (preferred).
