Responsibilities
- Perform direct HIV/STI counseling and testing services on inmates, including specimen collection for submission to the State Laboratory.
- Provide court‑ordered HIV/STI/HCV counseling and testing services when required.
- Act as liaison between the Florida Department of Health, St. Lucie County jail, Department of Juvenile Justice, and other detention facility officials and subcontractors.
- Attend meetings related to service coordination and special events for the Jail Linkage Program.
- Facilitate regular HIV/STI/HCV prevention education classes for inmates and distribute evaluation forms and pre‑/post‑tests.
- Complete Jail Linkage monthly and quarterly reports, case‑management forms, laboratory request forms, and travel reimbursement forms in accordance with CHD policy.
- Perform original post‑test counseling, partner counseling, referral services, and linkage to care for individuals testing positive.
- Communicate with public and private clinics, hospitals, laboratories, and other providers to obtain client information and arrange medical evaluations.
- Assist with linkage/re‑engagement activities, ensuring individuals are linked to care within 30 days and completing required fields in the HIV/AIDS Linkage Module and STARS.
- Coordinate with local Regional Linkage Coordinator, HIV/AIDS surveillance and STD program staff on Data to Care, MMP, MHS, and Ryan White re‑engagement line lists.
- Identify barriers to care and assist HIV‑positive individuals with transportation, medication, insurance, and other practical aspects.
- Perform outreach events, community screenings, and rapid testing techniques as needed.
- Collaborate with DOH or community partners for outreach counseling and education at community events.
- Maintain accurate records and reports, foster client‑led support groups, and develop materials to improve HIV care quality and efficiency.
- Ensure client confidentiality and comply with DOHP 50‑10 Information Security & Privacy policy.
- Submit logs and monitoring reports to supervisor and participate in department and unit meetings.
Required Knowledge, Skills, and Abilities
- Ability to travel within area 15 and possess a valid driver’s license.
- Case‑management experience and phlebotomy training.
- Self‑motivated, organized, and effective communication skills.
- Proficiency in Microsoft Office (Word, PowerPoint, Excel, Outlook) and PowerPoint presentation skills.
- Comfortable interacting with individuals and groups and delegating tasks.
- Experience working with diverse communities and planning coordinated work assignments.
- Completion of Linkage to Care (ARTAS) and Disease Intervention Specialist training.
Qualifications
Minimum
- Certified or willing to become certified in phlebotomy.
- Willing to travel out of county and between neighborhoods.
- Available to work before and after hours, weekends, and during emergencies.
- Minimum 1‑year case‑management or patient care coordination experience.
- Valid driver’s license.
- Pass required background screening for St. Lucie County Jail, including fingerprinting and criminal history checks.
Preferred
- Certified phlebotomist.
- Experience with HIV/STD programs and related work.
- Quality assurance, file reviews, and report development experience.
- Experience with detention facilities, drug rehab, mental health facilities.
- Advanced Microsoft Office skills.
- Case‑management experience or caseload management.
- Risk‑reduction education on HIV/STD and community event experience.
- Work with marginalized, low‑income, and disenfranchised communities.
- Bilingual (Spanish and/or Creole) preferred.
