Job Overview The Medical Social Worker (BSW) will work full‑time, day shift, at Franklin Memorial Hospital (180 Floyd Ave, Rocky Mount). The role focuses on assisting patients and families in managing personal and environmental challenges that may hinder their medical care or utilization of benefits. Responsibilities Assist patients and families with psychosocial support, education and referrals to address disease and life‑limiting illness. Conduct psychosocial assessments to identify emotional, social and environmental strengths and problems affecting health management. Plan and coordinate care with the health‑care team, including transition plans, to promote optimal health behaviors and functioning. Monitor, evaluate and record client progress, adjusting plans as needed and documenting quality, timeliness and confidentiality of records. Track healthcare resource utilization (ED, urgent care, admissions, office visits) and collaborate to reduce unnecessary use. Arrange and coordinate assistance/referrals for unmet patient/family needs (social services, financial, insurance, medications, DME, transportation). Advocate on behalf of patients and participate in care‑transition planning and post‑discharge follow‑up. Collaborate with professionals and community agencies to assess needs, build resources, and provide access to services such as financial aid, legal help and housing. Maintain knowledge of benefit plans, regulatory guidelines, and resources to maximize utilization and cost effectiveness. Adhere to documentation standards and maintain accurate records and statistics per department and medical‑center policies. Continuously update knowledge of social work and healthcare developments and seek further enhancement of job‑related skills. Identify social problems and advocate for systemic change, providing education and self‑management strategies to patients and families. Qualifications Bachelor’s degree in social work (BSW) required. Social work experience in a health‑care setting preferred. Strong critical‑thinking and creative problem‑solving skills to move patients effectively through the continuum of care.
