JOB OVERVIEW • Job Title: Internal Medicine/Family Medicine • Job Type: Locum Tenens • Location: Ohio - Mentor, Lorain, Willoughby Hills, Brunswick, Fairlawn • Service Setting: Outpatient Clinic • Coverage Type: Clinical Only • Coverage Period: 10/05/2026 – 12/31/2026• Shift Schedule: Monday – Friday, 8:00 AM – 5:00 PM - Minimum Two Weeks Per Month PATIENT INFORMATION • Patient Demographics: Adolescents, Adults, and Geriatrics • Patient Volume/Census: 15–20 Patients Per Day • Case Mix: Outpatient Primary Care FACILITY INFORMATION • EMR System: Epic • Office Equipment Available: Standard Outpatient Clinic • Specialty Backup Available: Cleveland Clinic Network • Support Staff Available: APPs Available at Most Sites PRIVILEGES & COMPLIANCE • Credentialing Timeline: TBD COMPENSATION & BENEFITS • Rate: TBD • Travel, Lodging, and Malpractice Insurance: Covered REQUIRED PROCEDURES • Adult and Adolescent Primary Care • Preventive Medicine • Chronic Disease Management JOB REQUIREMENTS • Active Ohio Medical License • Board Certified or Board Eligible in IM or FM • Active DEA • Certifications: Ohio CSR Required • Experience: Outpatient Primary Care Experience • Other Qualifications: Internal Medicine physicians must see all ages; NPDB Self-Query within previous 6 months required DUTIES & RESPONSIBILITIES • Provide outpatient primary care services • Evaluate, diagnose, and manage acute and chronic conditions • Document patient encounters in Epic • Collaborate with APPs and multidisciplinary teams • Float among assigned Cleveland Clinic outpatient locations
