Life Skills Coordinator

Reports to: Agency Manager Revised: 09/23/2026 Job Summary: The Life Skills Training Coordinator will implement and oversee programs that equip individuals often from underserved or vulnerable populations, with the practical skills needed for independent living, community integration, and personal growth. The role combines direct coaching, program coordination, and collaboration with stakeholders to ensure that our members achieve their goals and improve their quality of life. Qualifications/Educational Requirements: Bachelor’s degree in social work, education, psychology, or related field, or equivalent experience. At least one year of relevant training experience in personal assistance agency or related health programs (non-medical home care, nursing facility). Proven experience working with individuals with disabilities, trauma survivors, or those recovering from addiction. Excellent oral and written communication and presentation skills including ability to effectively communicate with clients. Professional demeanor and appearance. Proficient skills to promote excellent client relations and customer skills while maintaining positive attitude. Strong interpersonal, communication, and time management skills; ability to work independently and collaboratively. Excellent organizational and time management skills. Ability to work independently and to carry out assignments to completion within parameters of instructions given, routines and standard accepted practices. Proficiency with office software and electronic record-keeping systems. Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodation. Serve as the agency’s internal resource for all Life Skills Training Coordination: Program Design & Delivery : Develop and deliver individualized life skills training plans aligned with our members goals, covering areas such as personal hygiene, housekeeping, cooking, budgeting, time management, social benefits, community integration and problem-solving. Direct Support & Coaching : Provide regular, as-needed life skills support to assigned members transitioning from a more restrictive community setting to a less restrictive community setting, or experiencing a change in life circumstance, using structured frameworks and program trackers. Support agency’s workforce to provide Life Skills Training that may be used for up to 365 days following the first day the service is provided. Community Integration : Facilitate access to community resources, including counseling, recovery programs, employment services, and social activities, to promote independence and social connection. Collaboration : Work closely with care teams, case managers, healthcare professionals, and other service providers to ensure comprehensive, coordinated support. Resource Navigation : Guide members in identifying and utilizing available community services, and encourage constructive communication with case workers, and other stakeholders. Documentation : Maintain accurate, timely, and confidential case notes, progress reports, and program evaluations in compliance with Vantage Plus Inc policies. Conflict Resolution & Safety : Assist with conflict resolution, ensure members safety, and implement remedial actions as required. Program Evaluation : Monitor members’ progress, adjust training plans as needed, and evaluate program effectiveness.
