Health Coach II

The Health Coach II will serve as an integral part of our onsite primary care team, responsible for helping patients improve their health by providing them with tools and motivation to make healthier choices, specifically related to nutrition, therapeutic menus, and diet plans. The Health Coach II will serve as patients' first point of contact when it comes to health and wellness matters within the clinic and beyond, and serves as an advocate for patients in all areas related to health and wellness. Responsibilities Create customized therapeutic menus and diet plans for patients with diabetes, obesity, hypertension, and other chronic diseases. Provide medical nutrition therapy to individuals and in a group setting to help manage diabetes and obesity. Train staff on topics related to nutrition and food service. Build long-term relationships with patients who visit the wellness center. Conduct group and individual diabetes self‑management education sessions using evidence‑based protocols. Take patients' vitals and document their visits with the medical provider. Reinforce the medical provider’s recommendations and help patients create plans to adhere to those recommendations. Participate in onsite and offsite outreach efforts to help engage patients in health and wellness. Provide outpatient diabetes and self‑management training and education to type1, type2, and gestational diabetic patients. Education includes but is not limited to signs/symptoms and respective treatments for hyperglycemia, hypoglycemia, DKA, and HHNK. Provide lifestyle and nutritional counseling to patients taking a GLP‑1 for weight loss. Plan, develop, and implement individual and group diabetes education for adults, pediatrics, and their families and caregivers. Instruct patients in the evidence‑based Standards of Care for diabetes, obesity, hypertension, and other chronic diseases. Monitor and evaluate short and long‑term patient responses to therapeutic interventions in collaboration with clinicians. Documentation of patient interactions in EMR. Identify ways to improve the care of patients and make recommendations to leadership. Travel may be required to both Delano and Lost Hills clinics. This position will require visiting remote locations on our mobile clinic. Qualifications Bachelor's degree in Nutrition, Dietetics or related field. 3+ years of nutritional services experience, preferred. Experience creating and customizing therapeutic menus and diet plans. Experience training food staff on topics related to Nutrition and Food Service. Experience providing medical nutrition therapy to individuals and in a group setting to help manage diabetes and obesity. Written and verbal fluency in Spanish, required. Exceptional verbal and written communication skills. Knowledge, Skills & Abilities Knowledge of community wellness, nutrition, and physical fitness. Knowledge of diabetes progression and treatment, preferred. Outstanding communication skills and customer care. Time management and excellent organizational skills. Proficient in Microsoft Word, Excel, PowerPoint Point and Outlook. Self‑directing with the ability to work with little direct supervision. Demonstrate effective oral and written communication skills. Benefits Pay Range: $40.00- $45.00/hour. Final compensation will be dependent upon skills & experience.
