Job Description SummaryWe are seeking a Patient Care Specialist II to join our Home Health team. In this remote role, you will coordinate front office operations, scheduling, patient communications, and documentation activities that support efficient care center operations. You will utilize your knowledge of clinical support workflows to address routine patient access inquiries, maintain accurate patient records, manage appointment scheduling, and facilitate effective communication between patients, providers, and care teams.The Patient Care Specialist II performs standard operational processes under moderate supervision, applying established knowledge and skills to tasks that may occasionally require interpretation or deviation from standard procedures. This role follows defined processes and guidelines, with decisions directly impacting team productivity and patient experience.
