Home Health Registered Nurse (RN)

Location: Boyd, Carter, Greenup, Elliott & Lawrence Counties, KY
Job Type: Full-Time Contract
Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care and comprehensive case management for patients in the home setting. This role focuses on delivering high-quality, patient-centered care while collaborating with physicians, caregivers, and interdisciplinary teams to improve health outcomes and support patients throughout their care journey.

Schedule & Benefits

• Shift: Monday–Friday, 8:00 AM – 5:00 PM
• Guaranteed Hours: 40 hours per week
• On-Call Rotation: Every 3 weeks
• Mileage Reimbursement: $0.43 per mile
• Productivity Requirement: 30 productivity points per week

Service Area

Patients are located throughout:

• Boyd County
• Carter County
• Greenup County
• Elliott County
• Lawrence County

You are not expected to cover every county in a single day.

Responsibilities

• Perform comprehensive nursing assessments for home health patients.
• Develop, implement, and continuously evaluate individualized plans of care.
• Collaborate with primary care providers and interdisciplinary teams to ensure appropriate treatment plans are followed.
• Coordinate care transitions between healthcare settings.
• Assess patients for appropriate, cost-effective levels of care.
• Arrange non-medical support services, including transportation and community resources, to improve treatment compliance.
• Coordinate with physicians, social workers, discharge planners, therapists, and other healthcare professionals.
• Monitor patient progress and adjust care plans as needed.
• Maintain detailed clinical documentation using HomeCare HomeBase (HCHB).
• Identify opportunities for health promotion, disease prevention, and patient education.
• Recognize potential complications early and intervene to improve patient outcomes.
• Advocate for patients while maintaining strict confidentiality and privacy standards.
• Ensure care is delivered in accordance with physician orders, agency policies, and regulatory requirements.

Required Qualifications

• Active Kentucky Registered Nurse (RN) License or Compact RN License
• Minimum 1 year of Home Health experience
• Previous experience using HomeCare HomeBase (HCHB)
• Strong clinical assessment, care planning, and documentation skills
• Excellent communication and care coordination abilities
• Valid driver's license, reliable transportation, and current auto insurance

Preferred Qualifications

• Associate's or Bachelor's Degree in Nursing
• Case Management certification (preferred)
• Experience coordinating complex patient care across multiple healthcare settings
• Ability to analyze clinical information and make sound nursing decisions
