Job Summary The LCSW/Behavioral Health Specialist will assist, coordinate, and manage the needs of Behavioral Health patients who are hospitalized, maintaining the Behavioral Health High Priority Patient registry (BH-HPP). This role adheres to departmental goals, quality patient care compliance, and state licensure requirements. Responsibilities Conduct timely assessments, diagnoses, treatment planning, and interventions for patients, including readmitted patients and those at high risk for readmission. Identify and implement appropriate community resources; assess behavioral, psychosocial, financial, family, palliative care, and home safety issues. Collaborate with general medical providers (PCPs) to educate them regarding behavioral health disorders and available treatment options. Assist with obtaining community resources and refer patients to outside agencies such as Medicaid, meals, medication assistance, and housing. Manage patients on the HPP Behavioral Health Registry through casework, utilization management, and discharge planning to promote optimal functioning post-discharge. Act as a behavioral health liaison between PCPs and patients to improve coordination of services and information flow. Provide self‑management support and ongoing phone contact with patients. Coordinate, review, and maintain daily logs for reporting purposes and weekly review. Maintain communication with other healthcare team members through attendance at appropriate meetings. Provide clinical consultation in an integrated health care environment addressing behavioral health diagnoses and socio‑cultural determinants. Ensure timely, accurate, thorough, and appropriate documentation per JenCare policies and billing guidelines. Maintain LCSW state licensure and attend required training seminars to keep licensure current. Knowledge, Skills, and Abilities Extensive knowledge of counseling theories, therapies, and techniques for individual, family, and group treatment. Familiarity with evidence‑based behavioral health methods related to mental health and substance abuse issues in a primary care setting. Understanding of normative changes related to aging and older adulthood. Proficiency in assessing behavioral health challenges in frail, high‑risk older adults. Knowledge of advance care planning, palliative care, and skills in addressing advance planning with patients and families. Ethical practice consistent with JenCare policies and professional standards. Skill in communicating with and providing psychosocial support to patients with cognitive impairment. Ability to conduct psychosocial interventions with challenged caregivers/family systems of high‑risk patients. Effective utilization of community‑based resources. Sensitivity and ability to provide care to people from diverse cultural backgrounds. Proficiency in written communication; documentation must be clear, concise, and compliant with JenCare policy. Teamwork skills in care coordination with patients, family systems, JenCare staff, and external providers. Excellent interpersonal, communication, and critical‑thinking skills. Education and Experience CSW (Certified Social Worker) required. Bachelor’s degree required. Minimum of two (2) years of experience working with behavioral health clients, providing direct evidence‑based assessment, diagnosis, treatment planning, intervention, and education to patients or families. Ability to work autonomously. State registration may be required if applicable. Pay Range Salary: $54,358 – $77,655 annually.
