We support patients and their families with dignity, comfort, and love./pp$5,000 sign on bonus!/ppWhat youll do as a hospice RN / RN case manager:/pulliBe the primary point of contact for patients and families, guiding them through every step of their hospice journey./liliDeliver and document skilled, hands-on nursing care based on each patients individualized plan of carein their home, assisted living, or wherever they call home./liliPerform initial and ongoing assessments to monitor patient condition and adjust care as needed./liliCollaborate with an interdisciplinary teamincluding physicians, LPNs, CNAs, social workers, chaplains, and othersto build and refine personalized care plans./liliProvide emotional, physical, and spiritual support not just to the patient, but to their loved ones as welloffering education, comfort, and bereavement guidance./liliTeach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance./liliCommunicate important updates in patient condition to the care team, ensuring timely and appropriate interventions./liliMaintain accurate, up-to-date clinical documentation and follow infection control and compliance standards./liliSupport the organizations quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient./liliHelp patients and families understand and navigate topics such as:/liulliMedication administration/liliHospice philosophy and services/liliSymptom and pain management/liliEnd-of-life processes and expectations/li/ul/ulpAbout you:/ppQualificationswhat youll bring:/pulliActive RN license in the state of employment (or eligible to obtain)./lili1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields./liliA genuine hospice heart: compassionate, empathetic, and patient-centered./liliComfortable providing care in diverse settings, including private homes and facilities./liliStrong communication and critical thinking skills in emotionally sensitive situations./liliValid drivers license, auto insurance, and reliable transportation./liliCPR certification required./li/ulpPreferred experience (not required):/pulliHospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)/liliAdmissions RN, wound care, float pool, travel nursing/liliExperience with terminally ill patients or serious illness support/li/ulpCompensation may vary within the salary range provided based on several factors including but not limited to a candidates location, experience, education, skills, licensure, certifications and department equity.
