Physical Therapy Assistant

Apollo Health and Rehabilitation Center is seeking a skilled and compassionate Physical Therapy Assistant to join our team of dedicated healthcare professionals. As a Physical Therapy Assistant at Apollo, you will play a vital role in providing high-quality care to our residents, helping them to achieve their rehabilitation goals and improve their overall quality of life.

Responsibilities:

• Provide treatment and documentation for residents assigned to caseload
• Provide appropriate physical therapy interventions based on resident evaluations, collaborating with other patient team personnel to maintain standards for professional physical therapy practice and interdisciplinary teamwork
• Observe and assess treatment effects, accomplishment of short- and long-term goals, and recommend changes to the Physical Therapist as needed
• Perform an appropriate screening on each patient as per policy, and perform accurate and timely GG scoring
• Follow resident-specific plans of care for physical therapy, and perform other duties as assigned

Qualifications:

• Current Florida professional licensure as a Physical Therapy Assistant
• Must have long-term care experience, knowledge of PDPM, Section GG, and LTC program development
• Familiarity with Net Health and Point Click Care

• Day or evening shifts

License/Certification:

• Requires active Florida Professional Physical Therapy Assistant license

Requirements:

• 1 year of experience
