- The Nurse Practitioner (NP) will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Visits will be completed in-home and/or community base services such as assisted living facilities, skilled nursing facilities or group homes or other delegated locations.
- Diagnosis & Treatment Formulate accurate diagnoses and develop individualized treatment plans using evidence-based guidelines.
- Chronic Disease Management Monitor and manage chronic conditions such as diabetes, hypertension, and cardiovascular disease, ensuring optimal control and patient education.
- Medication Management Prescribe and adjust medications as needed, while educating patients on proper use and potential side effects.
- Preventive Care Promote wellness through immunizations, screenings, and lifestyle counseling to prevent future illness.
- Patient Education Empower patients with knowledge about disease management, healthy living, and self-care strategies.
- Collaborative Care Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and seamless care transitions.
- Primary Care Provider of Record Serve as the designated primary care provider for assigned patients.
- Documentation Maintain accurate and timely EMR documentation, with all visit notes completed within 48 hours.
- Bedside Manner Foster trust and rapport through empathetic communication and genuine care.
- Follow-Up Care Conduct regular follow-up visits to evaluate treatment effectiveness and make necessary adjustments.
- Quality Assurance Participate in quality improvement initiatives and maintain compliance with healthcare standards and regulations.
- Active, unrestricted licensure as a Provider (Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant) in Arizona.
- Active Drug Enforcement Administration (DEA) license.
- Current Basic Life Support Certification for healthcare providers.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and patient education abilities.
- Proficiency in electronic medical record (EMR) systems.
- Valid driver's license for a minimum of 3 years with a clear driving record (no DUIs or felony driving convictions in the last 7 years).
- Valid proof of sufficient car insurance coverage and access to reliable transportation.
- Ability to pass a comprehensive background check and drug screen.
- 1+ year of experience in primary care or a related field (family practice, general practice, or outpatient care) in the Arizona market.
