Schedule Flexible shifts available Monday through Sunday Self-scheduling to fit your lifestyle Minimum weekly or bi-weekly hourly commitment Primary Responsibilities Perform comprehensive annual health and post-discharge assessments in patient homes across Howard County, including Kokomo, Greentown, and Russiaville Evaluate medical histories, current health conditions, and medication regimens Educate members about their medical conditions and medications to promote self-management and understanding Identify gaps in care, providing actionable recommendations to address potential risks No requirement to develop treatment plans or prescribe medications Desired Qualifications Active Nurse Practitioner license in Indiana Board Certification via AANP or ANCC Current CPR certification (BLS required) MSN degree from an accredited nursing program At least six months experience as a Nurse Practitioner preferred If you have a passion for patient education, independent clinical assessment, and the flexibility to create your own work schedule, this is an excellent role to help drive positive healthcare changes from the comfort of patients homes.
