LPN/RN Needed - Skilled Nursing
Job Summary:
Orrville Skilled Nursing and Rehabilitation is seeking a dedicated and compassionate LPN/RN to join our team of healthcare professionals. As a valued member of our staff, you will have the opportunity to provide individualized, compassionate, and innovative healthcare services to our guests. About the Role:
As an LPN/RN at Orrville Skilled Nursing and Rehabilitation, you will be responsible for:
Providing direct patient care, including administering medications, conducting vital signs, and performing various medical procedures
Developing and implementing individualized care plans tailored to each guest's unique needs and goals
Collaborating with other healthcare professionals to ensure high-quality, patient-centered care
Maintaining accurate and detailed records of patient care and progress
Providing emotional support and education to guests and their families
Responsibilities:
Provide nursing care to guests in accordance with established policies and procedures
Conduct initial and ongoing assessments of guests' physical and emotional needs
Develop and implement individualized care plans, in conjunction with other healthcare professionals
Monitor and report changes in guests' conditions to healthcare professionals and family members
Participate in multidisciplinary care conferences to discuss patient progress and goals
Maintain accurate and detailed records of patient care and progress
