Director of Social Work

Job Summary Executive Search Network/Platinum Staffing is seeking a highly skilled and experienced Director of Social Work to join our team at a skilled nursing, long‑term care facility located in Suffolk County, New York . As a key member of our team, you will be responsible for providing exceptional social services to our residents, as well as leading and managing a team of social workers. Job Responsibilities Assist new residents and their families in the initial adjustment phase, as well as in the ongoing transition to institutional living Provide information to residents and/or family members on residents' rights and responsibilities Interview residents/families to obtain social history, and participate in discharge planning, development and implementation of social care plans and resident assessments Interpret social, psychological and emotional needs of the resident care team members, and involve the resident/facility in planning objectives and goals for the resident Refer residents/families to appropriate social services agencies as needed and arrange transportation to other facilities when necessary Qualifications Must have experience as a Social Worker in a Nursing Home Strong written and verbal skills Excellent communication skills Bachelor's degree in Social Work or related field Minimum 2 years of experience as a Social Worker in a Nursing Home Strong leadership and management skills What We Offer Competitive salary range of $100,000 - $110,000 annually Opportunity to work in a dynamic and supportive environment Chance to make a meaningful impact on the lives of our residents and their families How To Apply If you are a motivated and compassionate Social Worker looking for a new challenge, please apply to this exciting opportunity to join our team.
