Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Supervise and mentor students as assigned, providing a positive learning environment, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain active and unrestricted professional licensure and fulfill all state-mandated continuing education requirements
• Participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Indiana
• Must be an experienced clinician — new graduates will not be considered
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation standards compliant with state, federal, and insurance guidelines

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students
• Familiarity with performance improvement methodologies and quality initiatives
