Responsibilities

• Evaluate needs of assigned patients, establish goals, and implement interventions to promote independence with activities of daily living
• Interact professionally with patients, families, and the health care team; collaborate with physicians on care plans
• Develop and update a plan of care (POC), document during care in the electronic medical record, and ensure timely completion
• Provide supervision to OT/OTA staff and students; uphold professional standards and evidence-based practice per AOTA standards
• Maintain a clean, safe care environment and uphold patient privacy and confidentiality
• Assist with patient transfers/positioning and participate in ongoing professional development and in-service activities
• May participate in Saturday/Sunday rotation as applicable and contribute to ongoing quality improvement

Requirements

• Active Illinois Occupational Therapist license
• BLS (AHA) certification (active at time of submission)
• COVID-19 vaccination required; declinations will not be approved
• Minimum 1 year Home Health OT experience (required)
• OASIS and start-of-care experience strongly preferred
• Valid driver’s license and car insurance (required)
• Education: Degree in Occupational Therapy from an accredited program (as applicable per job notes)
