Nurse Practitioner (PRN) - In-Home Health Assessments

Responsibilities Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case‑management referrals, and follow‑ups, documenting all care in accordance with Matrix guidelines and protocols. Administer and order point‑of‑care and lab‑based testing as needed. Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate. Work with Primary Care Physicians to coordinate patient education and follow‑up. Provide services at members’ homes or Skilled Nursing Facilities. Benefits of Working at Matrix Flexibility – Visits are scheduled based on your availability from Sunday to Saturday, 8 am–7 pm. You can work during the day, evening, or weekend. Competitive Pay – Earn income per completed assessment: $90–$115. Monthly earnings range from $1,800 to $11,500 for 20–100 visits, varying by state rate. Other Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits. Education and Support – Comprehensive onboarding training, a preceptor, 1:1 feedback, and free continuing education access. Additional Licensure – Company support for obtaining licensure and credentialing in neighboring states when needed. Full‑time Opportunities – PRN providers can transition to full‑time roles and receive a sign‑on bonus. Qualifications Master’s degree or equivalent experience and satisfactory completion of NP licensure. Current NP licensure in the state of practice, including prescription authority or eligibility for prescriptive authority. Board certification (AANP, AACN, or ANCC) in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties. Current BLS, ACLS, or CPR certification. One year of NP experience preferred; new graduates encouraged to apply. Valid state driver’s license and required automobile insurance coverage. Liability coverage: $100,000 per person / $300,000 per incident (bodies injury) and $50,000 property damage. Deductible: $1,000 or less for collision and comprehensive. Strong computer skills and familiarity with employee health/medical record software. Excellent verbal and written communication skills.
