Description

• Conduct patient evaluations in inpatient or outpatient settings and create personalized treatment plans
• Implement physical therapy interventions focused on enhancing mobility, strength, balance, and overall function
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and families through therapeutic goals, discharge planning, and home exercise programs
• Document patient evaluations, treatment intervention, and progress according to facility and regulatory standards
• Engage in interdisciplinary team rounds and conferences to provide coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

Certifications

• Licensed as a Physical Therapist in the state
• CPR certification preferred
• Additional certifications (such as neurological or geriatric specialties) are advantageous but not mandatory
