Minimum Qualifications Associate degree in Physical Therapy from an accredited educational program. Active Illinois PTA license required to start the assignment. BLS certification from the American Heart Association or American Red Cross may be required. Experience working in a skilled nursing facility preferred. Current documentation of immunizations and TB screening as required by facility policy. Ability to work with geriatric and medically complex patients, including experience with transfers, gait training, and therapeutic exercise preferred.

Assignment Details Facility type: Skilled Nursing Facility. Shift: Days. Shift hours: 08:30–16:30. Work pattern: Five 8-hour shifts per week. Start: ASAP. Assignment length: 15 weeks. Guaranteed hours per week: 32.
