PT-Physical Therapist (Home Health)
100-120k (DOE)
Full Time Permanent Position
Kitsap County, WA

Position Summary:

The Physical Therapist works within the context of a dynamic interdisciplinary team. The Physical
Therapist’s goal is to optimize functional mobility and activities within the home setting and prepare for
community reentry, while promoting home safety for the patient and caregiver(s). The Physical Therapist
is responsible for the evaluation, planning, directing and administering of physical therapy modalities of
treatment as prescribed by a licensed physician; administers treatments and physical agents as
prescribed by a referring physician in an effort to restore function and prevent disability following injury,
disease or physical disability; and, assists patients to reach their maximum performance and level of
functioning, while learning to live within the limits of their capabilities.

Requirements

• Must hold licensure as a Physical Therapist through the appropriate State Board of Physical
  Therapy. Valid driver's license with maintenance of safe driving record and an automobile that is insured in
  accordance with state requirements.
• Experience in gerontology and home care highly desired.
