Hospice RN Care Manager Lead Compassionate Teams

Gentiva Hospice is seeking a dedicated Patient Care Manager to lead clinical operations and coordinate patient care in a home-based hospice setting. Reporting to executive leadership, you will guide an interdisciplinary team, ensure regulatory compliance, and drive quality care delivery across the location.

You will triage referrals, manage visits, mentor staff, and uphold compassionate, compliant care for patients and families, with strong emphasis on documentation and performance improvement.
