Position: Director of Social Services
Location: Delray Beach, FL
Shift: Full-Time | Monday–Friday
Salary: $65,000–$85,000 annually

West Delray Nursing and Rehabilitation Center is seeking an experienced and compassionate Director of Social Services to lead our social services department and support the emotional, social, and overall well-being of our residents.

Responsibilities

• Oversee the Social Services Department and ensure regulatory compliance.

• Complete resident and family assessments and develop individualized care plans.

• Coordinate discharge planning, referrals, and community resources.

• Advocate for residents and provide support to residents and families.

• Collaborate with nursing, therapy, and administrative teams.

• Lead, train, and mentor social services staff.

• Participate in surveys, inspections, and quality assurance initiatives.

Qualifications

• Bachelor’s Degree in Social Work (BSW) or related field required; MSW preferred.

• Minimum 2 years of SNF/social services experience .

• Leadership experience strongly preferred.

• Excellent communication, organizational, and problem-solving skills.

• Strong commitment to resident-centered care.
