Your recruiter will provide the most up-to-date details during the hiring process._ **How you'll make an impact in this role** + Assess participants' social, emotional, economic, and environmental needs, and develop appropriate intervention plans; home visits may be required. + Conduct interviews, prioritize needs, and accurately document case activities and progress. + Collaborate as a team-oriented department with managers, other departments, agencies, and caregivers to ensure alignment with treatment plans and continuity of care. + Connect participants and caregivers with community resources, supporting discharge planning and ongoing post-care needs. **What minimum requirements you'll need** Licensure / Certification / Registration: Preferred Credential(s): + Drivers License. + Masters Level Social Worker. Education: + Master's degree in Social Work required.
