We are looking for a qualified individuals with a passion for working with older adults to join our team! Experience in Social Work or Registered Nurse preferred. You will play a crucial role in assessing clients in their environment for unmet needs and developing individual care plans in collaboration with patients family, physicians and financial and legal team. Operating with professional expertise and deep care for patients, you are a natural problem solver and self-starter. You enjoy working in fast-paced environments that afford you the autonomy to bring your best. Responsibilities
- Quarterback the patient's needs to make sure they are met.
- Participate in patients plans of care. Develop "To Do" list outlining priorities for health and safety issues
- Assess the psychosocial status of the patients as related to their illness
- Schedule regular visits to oversee plans of care and those providing services such as home health aides.
- Plan interventions based on patient's needs and findings
- Maintain accurate and up-to-date records
- Masters degree in social work
- Minimum one year of experience in health care and social work Valid drivers license
- Psych experience is a plus
- Compassion, desire to learn and flexibility necessary.
