This full-time Hospice Physician position leads the Capital Caring DC hospice team and includes in-person home visits within the District of Columbia and daily remote patient management. The Hospice and Palliative Medicine (HPM) Physician is a member of the Active Medical Staff of Capital Caring Health and serves patients across the hospice and palliative care continuum, including home, facility-based hospice, and inpatient hospice. Responsibilities Provides medical management across all care settings within Capital Caring Health at the direction of the General Manager with support from the Neighborhood Lead Physician. (H/PC) Effectively complements attending physician care. (H/PC) Acts as a medical resource person to the IDT/IDG. (H/PC) Assures overall continuity of medical services provided to hospice patients. (H) Assures that the patient receives appropriate measures to control uncomfortable symptoms. (H/PC) Serves as a medical resource to staff, patients, families, and attending physicians regarding pain and symptom control management. (H/PC) Ensures the provision of direct medical services to patients either directly or through arrangement, as appropriate, in the absence of the patient’s attending physician. (H) Attends IDT/IDG conferences; actively participates in interdisciplinary team meetings; arranges for alternative physician coverage if unable to attend the entire meeting. (H) Participates in plan of care development and the management oversight of medications and treatment. (H/PC) Documents care in the patient’s clinical record with evidence of progression of the end-stage disease process. (H) Provides triage, after-hours, and weekend coverage for region and hospice inpatients within the after-hours and weekend coverage system. (H/PC) Provides palliative medicine consultation for pain and symptom management for non-hospice patients, as requested by attending physicians, in home, hospital, or long-term care settings. (PC) Consults with IDT/G representatives; amends the plan of care as necessary to ensure alignment with palliative care. (H/PC) Is readily available for hospice visits for patients needing a palliative medicine consultation to guide care, as determined collaboratively by the hospice medical director, the interdisciplinary team, and/or the attending physician. (H/PC) Acts as a liaison to physicians in the community. (H/PC) Assumes Attending Physician responsibility for patients when needed to assure 24-hour physician coverage. (H) Educates and develops relationships with referral sources. (H/PC) Participates in collaborative research projects as possible. (H/PC) Organization and Compliance Re-certify patients, as appropriate, for continuation of Medicare Hospice benefit at appropriate levels of care. Adheres to clinical algorithms and completes documentation within specified time frames. Prepares thorough documentation of all clinical encounters to support charges. (H) Maintains current knowledge of hospice and palliative medicine research and trends; maintains continuing education as required for licensure; attends at least one hospice/palliative care educational program per year and maintains a professional development plan for ABMS certification in Hospice and Palliative Medicine within 24 months of employment (or other appropriate competence evaluation). (H/PC) Participates in QAPI activities, including regular medical record reviews and peer review; seeks to improve care practices to comply with regulatory and accreditation standards. (H/PC) Completes facility and payer credentialing in a timely fashion – not more than two (2) weeks after receipt. (H) Documents in the patient chart and forms to provide a written record of assessments and plans of care; ensures availability of information to team members. Complies with applicable laws and does not discriminate on protected status. (H) Reviews and develops protocols for treatment and proposes current intervention options. (H/PC) Participates in resolution of interpersonal conflict and ethical concerns. (H/PC) Participates in development and updating of patient care policies and emergency procedures when requested. (H/PC) People and Communications Participates in medical staff activities including weekly fellow teaching and related activities. (H/PC) Demonstrates knowledge in communication and counseling patients and families in end-of-life issues. (H/PC) Encourages coordination and communication with the patient’s attending physician; emphasizes customer service with regard to the attending physician and referral sources. Exhibits consultant etiquette by contacting attending physicians as needed. (H/PC) Educates and increases knowledge and skill levels of staff, trainees, or visitors in caring for chronically ill/terminally ill patients and families; oversees trainees to optimize the educational experience. (H/PC) Participates in performance improvement programs; works with IDT/IDG as a team. (H/PC) Provides consultation and education to colleagues and attending physicians about admission criteria for hospice and palliative care; available for IDT/G consultations on weekdays with urgent availability as needed. With leadership and stakeholders, promotes Capital Caring Health in hospice and palliative care concepts to strengthen community relationships and referrals. (Financial stewardship) Implements time-based billing procedures using the electronic medical record system and other methods; achieves productivity standards aligned with the organization’s model. (H/PC) Experience and Licensure The HPM Physician must have at least 3 years of related experience. Palliative Care Fellowship is preferred. Expertise in the medical care of terminally ill individuals is required. The HPM Physician must have an MD or DO from an accredited institution. Board certification in an American Board of Medical Specialties (ABMS) certified medical specialty with current recertification, and ABMS Board Certification in Hospice and Palliative Medicine with current recertification. Licensure and Certification Medical practice licensure from the District of Columbia, Maryland, and/or Virginia governing bodies. DEA and State Controlled Substances Registration in applicable jurisdictions. Required Abilities and Skills Ability to perform thorough nursing assessments including physical and psychosocial evaluations; provide evidence-based recommendations; function as a member of the IDT and CH/CPCC teams; manage multiple tasks and deadlines; work independently and in a team; demonstrate strong interpersonal and collaborative skills. Physical and Environmental Demands Ability to handle physical demands including lifting up to 50+ pounds; driving long distances; and working in a fast-paced environment. Includes exposure to blood or bodily fluids, and other related risks. Reasonable accommodations available. Work in a temperature-controlled building; occasional travel to alternate work locations.
