Compensation: $42,952.00 - $58,185.40 / year

Position Summary

Randstad is seeking a Case Manager to join the Center for Urban Community Services (CUCS). Supporting individuals and families experiencing homelessness, mental illness, and complex needs, this role delivers person-centered care to a dedicated caseload. Working on-site and in the field, the Case Manager provides outreach, crisis intervention, benefit coordination, medication support, and skill-building to promote housing stability and independence.

Key Responsibilities

  • Case Management & Engagement: Build relationships to create individualized service plans (ILPs); provide outreach, community appointment escorts (medical, legal, benefits), and medication self-administration support.

  • Benefits & Financial Admin: Assist clients with applying for and maintaining benefits (SSI/SSDI, Medicaid, SNAP), managing representative payee funds, and tracking goals.

  • Housing & ADL Support: Assist with move-in orientation (PackTite bed bug remediation, donation distribution), annual recertifications, inspection prep, and daily living skills (ADLs)—hygiene, cleaning, hoarding mitigation, and pest remediation.

  • Crisis & Operations: Provide crisis intervention, coordinate with emergency services and discharge planners, facilitate group activities, and log timely EHR progress notes to hit contact hour targets.

Qualifications & Requirements

  • Education: High School Diploma or GED required (Bachelor’s in Social Work, Psychology, or related field preferred).

  • Experience: No formal experience required (prior human services or supportive housing exposure preferred).

  • Competencies: communication, de-escalation skills, person-centered approach, and comfort with home/field visits.

  • Crisis Response & Operations: Deliver trauma-informed de-escalation, coordinate emergency care, run support/recreational groups, and maintain timely EHR progress notes for client contact targets.
