Hybrid Nonprofit Case Manager for Children and Families

Lakeshore Talent is seeking a case manager in Colorado Springs to provide essential services to children and families enrolled in Medicaid waiver programs. You will serve as the primary contact for assigned families, conducting assessments and coordinating care.
