LVN - On Call

Position Summary: Concord Post Acute is seeking a dependable and skilled Licensed Vocational Nurse (LVN) to join our team on an on-call basis. The LVN will provide direct nursing care to residents, support clinical staff, and ensure high-quality patient outcomes in a fast-paced post-acute care environment. This role requires flexibility, strong clinical judgment, and a commitment to compassionate care. Key Responsibilities: Provide direct patient care in accordance with established nursing standards and care plans Administer medications and treatments as prescribed by physicians Monitor and document patient conditions, reporting changes promptly to the RN or physician Assist with admissions, discharges, and transfers of residents Collaborate with interdisciplinary team members to ensure continuity of care Maintain accurate and timely documentation in compliance with regulatory requirements Ensure a safe, clean, and supportive environment for residents and staff Respond to call-offs and staffing needs on short notice Qualifications: Current, active LVN license in the state of California Current CPR/BLS certification Minimum of 1 year experience in a skilled nursing or post-acute setting preferred Strong clinical and assessment skills Excellent communication and interpersonal abilities Ability to work independently and adapt to changing patient needs Work Schedule: On-call, as needed Must be available to work weekends, holidays, and variable shifts as required Benefits: Competitive hourly rate of $38/hour Flexible scheduling Opportunity to gain experience in a supportive healthcare environment Equal Opportunity Employer Statement: Concord Post Acute is an equal opportunity employer and is committed to creating an inclusive environment for all employees.
