Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CA license required
• BCLS required
• HCHB required
• 1 year of recent skilled home health experience within the last 3 years
• Own transportation required

Assignment Details

• Monday through Friday, 8:00 AM to 5:00 PM.
• Geriatric caseload.
• May visit up to 6 patients per day.
• 1:1 patient ratio.
• Occasional travel radius of 50+ miles.
