Travel Skilled Nursing Facility Physical Therapist – Saint Charles, MN Critical Connection, Inc. is seeking a skilled nursing facility physical therapist for a 13‑week travel assignment in Saint Charles, Minnesota. The position starts ASAP and requires a 40‑hour per week schedule with 8‑hour day shifts. About the Opportunity The role focuses on restoring mobility, independence, and quality of life for short‑term rehab and long‑term care patients in a quality‑driven skilled nursing facility. Ideal candidates value 1:1 patient care, interdisciplinary collaboration, and meaningful functional outcomes. New graduates are welcome with mentorship available. What You’ll Do Evaluate and treat patients recovering from illness, injury, or surgery. Develop and update individualized plans of care (POC). Provide 1:1 skilled therapy to primarily Medicare patients. Collaborate with nursing, occupational therapy, speech‑language pathology, and rehab leadership. Document evaluations, progress notes, and discharge summaries. Educate patients and families on mobility, safety, and recovery. Qualifications Doctorate or Master’s Degree in Physical Therapy. Active (or pending) state PT license. Experience in skilled nursing facility preferred but not required. Strong clinical reasoning and communication skills.
