Pay: $2,546.40 weekly


Shifts: M-F 8 AM-5 PM (on call once a month for one weekend Sat/Sun)

This role focuses on case management within a home health setting, providing comprehensive care coordination and support for at-risk patients. The position covers multiple counties, including Johnson, Emanuel, Toombs, Montgomery, and others, offering a varied and engaging work environment.

Key Responsibilities of the RN Case Manager

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Identify and reassess cost-efficient, appropriate levels of care
• Coordinate non-medical support to ensure treatment compliance
• Coordinate care transitions as needed
• Work with physicians, social workers, and claims professionals for patient transition
• Engage specialty resources for optimal resolution
• Maintain detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate by protecting privacy and confidentiality

Requirements of the RN Case Manager

• Associates or Bachelor's Degree in Nursing or related field
• Current, unrestricted state Registered Nurse license
• Ability to analyze complex medical information and make rational decisions
• Case Management certification
