RN Case Manager: Discharge & Care Transitions

McLaren Health Care in Michigan is seeking a registered nurse to join the Case Management team as a patient care coordinator. You will assess patient needs, coordinate discharge planning, and collaborate with physicians and social services to ensure timely transitions. Daily work times are 8:00 am to 4:30 pm, shift Days, full-time with bi-weekly hours of 80.
