Candidates should be based within Durham County of North Carolina to cover the required field work. This position is PRN. Responsibilities Perform needs‑based eligibility determinations for North Carolina beneficiaries under the Community Alternatives (CAP) Program. Complete needs‑based assessments of level of care (LOC) to enable individuals to remain in or return to a home or community‑based setting. Process CAP SRFs (service referral forms); initiate RAIs for SRFs pending three days or less; issue Technical Denials for incomplete SRFs and document reasons in the communication log. Monitor the CAP SRF queue daily to ensure a maximum of 14 business days in the queue and check for duplicate beneficiaries. Complete CAP SRF 2nd level when necessary. Perform the CAPDA and CAPC comprehensive assessments. Receive cross‑training in up to four additional areas. Minimum hours: 16 per week. Training may span up to seven consecutive days, including up to four days of remote learning followed by three days of field training. Travel up to 120 miles per day for two assessments; if travel exceeds 120 miles, schedule will be reduced to one assessment. PRN staff are not scheduled on Saturdays. Work within Durham, NC and surrounding counties in Region 4. Maintain a two‑week advance schedule to allow sufficient time for appointment scheduling. Qualifications Required Qualifications Registered Nurse or LCSW license by the applicable state. Minimum of two years of nursing experience. This position requires travel up to a 60‑mile radius. Preferred Qualifications Knowledge of North Carolina Medicaid Clinical Policy 3K‑1 and 3K‑2, and 42 CFR Part 441 Subpart G. Knowledge of eligibility criteria for LOC and Waiver Participation. Minimum of two years’ experience in a home care setting preferred. Knowledge of standards of practice related to Medicaid waivers, HCBS programs, EPSDT, medical fragility, and level of care determinations. Knowledge and understanding of public sector services and supports. Understanding of services provided under the CAP waivers. Computer proficiency in Microsoft Excel, Word, and Outlook. Ability to use computer equipment and web‑based software to conduct work. Ability to interact with office staff to support workflows. Ability to interact with healthcare professionals, patients, families, and other supports. Ability to communicate effectively through spoken, written, and electronic media. Attention to detail and ability to prioritize and execute tasks timely. Ability to work independently without high supervision. Develop level of care recommendations based on clinical evaluations. Participate in training of CAP stakeholders as needed. Use person‑centered thinking and have competency in awareness of the needs of persons with disabilities.
