GENERAL SUMMAR
Y:Patient Care Coordinators work under the general direction of a physician or his/her designee, utilizing communication and management skills to coordinate seamless care delivery for patients in a surgical or non-surgical setting. Will act as a patient advocate to assure the patient's access to health care during their entry into and through the services of the clini
c.
ESSENTIAL FUNCTIO
- NS:Act as contact for clinic providers, patient and family, referring physicians, outside agencies, outside staff and general public by providing information regarding the clinic’s servic
- es.Act as liaison between the patient and the various clinic areas and ancillary services to coordinate activities involved in pre-operative planning and preparation or planned hospitalizati
- on.Coordinate pre-admission tests, pre-surgery appointments/tests, and procedures as delegated by the provider or nur
- se.Ensure a complete medical record is present at the time of patient visit, admission, and/or procedu
- re.Facilitate the flow of documents between patient, providers and paye
- rs.Communicate with health care team members regarding the status and/or completion of care coordination activiti
- es.If working in a specialty clinic that receives internal referrals, ensure that referrals are addressed in a timely manner under the direction of the clinic leadership te
- am.Work with financial services and establish procedures to ensure financial information is complete prior to appointments, procedures or admissio
- ns.Assist with scheduling surgeries and/or procedures or services needed in the ambulatory management of the patient’s ca
- re.Promote and protect general safety for patients/families/friends and co-workers through adherence to correct procedures, timely communication of messag
- es.Provide summary reports to clinic leadership on referral volumes and status (where applicabl
- e).Support development of clinic and divisional efficiency and effectiveness through participation in quality assessment and improvement activiti

REQUIRED QUALIFICAT
- IONS:1 year of experience in medical/healthcare setting or 2 years in a customer service
