Position Description: The process for determining if a Soldier is FMC is very detailed. There are developed timelines, so Soldiers and their families are not left in limbo receiving medical care. The process also provides a decision point, so mission commanders are not left in limbo either on the deployment capability of Soldiers assigned to their units. The case management team ensures the Soldiers receive the correct treatment for any injuries or illness. They do this through identifying medical appointment requirements for the Soldiers. All appointments and follow-ups are tracked in healthcare systems within the Medical Operations Data System which encompasses several applications and systems such as MEDPROS, Medchart or, eCase systems. The service member is tracked through the Integrated Disability Evaluation System (IDES). The service member is referred to the disability system within 1 year of being diagnosed with a medical condition that prevents deployment. The service member is referred to a medical evaluation board (MEB) and navigates the process within 140 days. If the injury or illness is severe the service member is referred to the Physical Evaluation Board and must navigate that process within 120 days. The entire determination process should be completed within 305 days from initial start of the MEB.

The Medical Assistant supports this process and others on the case management team.
Experience and Qualifications:
•\tShall be able to fluently read, write, speak, and fluently understand English.
•\tBe a U.S Citizen.
•\tKnowledge of medical terminology, simulation and integrating technology and ability or aptitude to learn and understand required integration software.
•\tMust be able to communicate both verbally and in writing.
•\tMust possess the ability to multi-task and work in an environment with constant interruptions and reorganization.
•\tHigh level writing, facilitation, problem solving, analytic, and briefing preparation communications/briefing skills, and minute taking skills, and other duties as assigned.
Duties:
•\tProvides support to the case management team as required
•\tShall complete medical and administrative charting in eCase NLT 48 hours after contact with Soldier, medical professional, or unit.
•\tRequest profile exams/evaluations and audiology exams at physical exam center.
•\tCoordinate Soldier treatment plans with appropriate medical facilities.
•\tSubmit case processing, medical documentation, and profile concerns to military/contacted provider.
•\tAttend PHA events as determined by the needs of the state and the individual units.
•\tEnsure those Soldiers referred by a provider are followed by the case management team.
•\tBecome familiar with the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment, to include the AR 40-501 and other applicable guidance.
•\tEnsure all medical documentation received from the Soldier and/or their medical team is received and placed in appropriate electronic record, as well as the Soldier’s hard copy record, NLT 5 days after receipt.
•\tReview and track health assessment (PHA, SRP) lab results and contact SM and educate on lifestyle modifications.
•\tUpload medical documents of case managed Soldiers into Health Readiness Record (HRR).
•\tStamp all medical documents for verification after uploading.
•\tIndex uploaded medical documents.
•\tProvide Records Custodian or delegate with all relevant medical/dental documentation received by units or Soldier.
•\tEnter immunizations, designation of pregnancy, eye exams, and medical warning tags into MODS.
•\tProvide all medical documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the contractor to the government. Copy of each supporting transfer document shall be provided to the COR at the end of each month no later than 7 calendar days at the end of the month.
•\tTrack medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation.
•\tLog into eCase any contact with SM (email, voice call, or personal interview) NLT 48 hours after contact.
•\tCommunicate with delegated government personnel to track compliance versus noncompliance with maintaining medical readiness.
•\tMonitor MEDPROS to ensure all scheduled services are completed/updated and current medical status is reflected.
•\tReport Medical Readiness of Soldier to the Commander (or his/her designated representative) during monthly MRC MACOM meeting and as requested in order to provide current case statuses.
•\tEnsure Soldiers with T3, T4, P3 or P4 profiles are case managed in the eCase module.
•\tECase records shall be reassigned within 5 working days of contractor personnel changeover.
Place and Performance of Services: The contractor shall provide services between the hours of 0730-1630, Monday through Friday except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Working Saturday or Sunday is possible under this contract and will depend on the mission requirements. Performance shall be at Camp Niantic, Building 804 located at 38 Smith St., Niantic, Connecticut 06357
