Discharge Planner

The Discharge Planner is responsible for performing case management and coordinating the discharge planning process for age-appropriate in-patients with chronic diseases and special health care needs. This includes working within the Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit. Key responsibilities include:

Maintaining a tracking system for referred patients for admission to other healthcare facilities.
Conducting chart reviews for requested services to facilitate return to facility or discharge home.
