Greek American Rehab & Care Center is seeking a compassionate and skilled Full-Time Speech-Language Pathologist (SLP) to join our rehabilitation team. The Speech-Language Pathologist will provide comprehensive evaluation and treatment services to residents with communication, cognitive, and swallowing disorders, with the goal of improving functional independence, safety, and quality of life.
Essential Responsibilities- Evaluate residents' speech, language, communication, cognitive-communication, and swallowing abilities.
- Develop individualized treatment plans based on each resident's clinical needs and goals.
- Provide skilled speech-language therapy and swallowing interventions.
- Treat communication disorders affecting speech, language, voice, fluency, and cognition.
- Assess and treat dysphagia and provide appropriate recommendations for safe swallowing and nutrition.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely evaluations, treatment notes, progress reports, and discharge documentation.
- Collaborate with physicians, nursing staff, occupational therapists, physical therapists, dietary staff, social services, and other members of the interdisciplinary team.
- Educate residents, families, and caregivers regarding communication strategies, swallowing precautions, and home programs.
- Participate in care planning and interdisciplinary meetings.
- Maintain compliance with applicable federal and state regulations, facility policies, and professional standards.
- Promote a safe, respectful, and resident-centered rehabilitation environment.
- Current Speech-Language Pathologist license in the State of Illinois .
- Master's degree in Speech-Language Pathology from an accredited program.
- Certificate of Clinical Competence (CCC-SLP) through ASHA, or eligibility as applicable.
- Current CPR certification, if required by facility policy.
- Experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred.
- Strong clinical, communication, organizational, and documentation skills.
- Ability to work effectively with residents, families, and an interdisciplinary healthcare team.
- Compassionate, dependable, and committed to providing high-quality resident-centered care.
